While we love it when you come by the Theatre Arts office to visit and pick up student forms, we know that sometimes our business hours don’t match up with your busy schedules. So, here are all the forms you may need!

Declare/Drop TA Major/Minor
Use this online form: https://theatre.uoregon.edu/undergraduate/resources/

Course Pre-Authorization Form.
Use this for Production Credits or any Independent Study Course. Fill out your information and then email the form to the instructor of the course. Click link above to access the form.

Change Theatre Arts Advisor Form.
Email completed form to Alohi, awright8@uoregon.edu. Click link above to access the form.

Course Waiver/Substitution Form.
Use this form if requesting to waive a TA Core Class or to request a substitution for a IIIA or IIIB requirement with another University of Oregon course from a different department. One substitution is allowed. Acceptable substitutions are listed on your Degree Audit in the IIIA and IIIB sections. After your TA advisor signs the form, please either bring your form to Alohi in the Theatre Arts office or email it to Alohi, awright8@uoregon.edu. Click link above to access the form.

Space Usage Request Form.
When you want to use Villard 102 & 104, Hope & Robinson Theatres, please fill out form and email it to Janet Rose, jrose@uoregon.edu. Approval for requests is not guaranteed. Click link above to access form.
